DR JAMES WOOD IS NOW ONLY SEEING PATIENTS AT OUR SISTER SITE (MOUNT PLEASANT CLINIC) AT 245 EGLINTON AVENUE EAST. 

HOWEVER YOU SHOULD STILL BOOK YOUR INPERSON APPT WITH HIM BELOW AND GO TO THAT CLINIC ADDRESS FOR THE APPT IF YOU WISH TO STAY UNDER HIS CARE.

ALTERNATIVELY IF YOU WISH TO REMAIN AT 500 YONGE ST INSTEAD, THEN YOU SHOULD BOOK YOUR APPT WITH DR ZENIB SARDAR INSTEAD, WHO HAS TAKEN OVER HIS PATIENTS HERE. 

(OTHERWISE, PLEASE EMAIL THE CLINIC IF YOU WISH TO CHANGE TO ONE OF THE OTHER DOCTORS AT 500 YONGE ST.)

***YOU MUST ARRIVE 15 MINUTES BEFORE your scheduled appointment time to register and test your vitals.***

***IF YOU ARRIVE MORE THAN 10 MINUTES AFTER YOUR SCHEDULED APPOINTMENT TIME, OR IF YOU CANCEL YOUR APPT WITH LESS THAN 24HRS' NOTICE, YOU WILL BE CHARGED A $60 PENALTY FEE AND REFUSED TO BE SEEN, NOR REBOOK WITH THE CLINIC, TIL THE PENALTY FEE IS PAID***
