***IF YOU ARRIVE MORE THAN 10 MINUTES AFTER YOUR SCHEDULED APPOINTMENT TIME, OR IF YOU CANCEL YOUR APPT WITH LESS THAN 24HRS' NOTICE, YOU WILL BE CHARGED A $60 PENALTY FEE AND REFUSED TO BE SEEN, NOR REBOOK WITH THE CLINIC, TIL THE PENALTY FEE IS PAID***

If this is your 1st appt it will only be a "meet and greet" and no specific medical concerns will be discussed. You MUST book subsequent appts to discuss any specific medical concerns and ONLY ONE ISSUE PER APPT.
